Position: Community Life Team Assistant

POSITION SUMMARY
The Community Life Team Assistant works with the Community Life Team Director to provide support and assistance to Community Life Activities, CityWalk Special Events and overall program support on an As Needed (PRN) basis. The Community Life Team Assistant is to be an expert in Customer Service who serves as a representative of CitySquare to the public during Events and Activities.
QUALIFICATIONS
Education: High school diploma or GED preferred.
Experience: Two+ (2) yeas experience providing outreach program assistance experience preferred.
Licensure/Certification:Crisis Intervention Training and CPR/AED Certification preferred.
Additional Requirements: Strong Customer Service Skills and Verbal De-escalation Skills required.
Position: Warehouse Driver
Department: Thrift Store
Status: Temporary
Schedule: Business hours, flexible hours as needed.
POSITION SUMMARY
The Truck Driver is responsible for the pickup of donated goods for the CITYSQUARE Thrift Store and other departments as necessary. The driver will coordinate and manage helpers, the transportation of donations, the loading and unloading of trucks, assist in the warehouse as necessary and be responsible for ensuring professional interaction with the donors. The driver is responsible for the safe operation of the truck, related equipment and the safety of the helpers on the truck at all times.
QUALIFICATIONS
Education: High school diploma or GED preferred.
Experience: One (1) year experience working as a truck driver or in warehouse preferred.
Additional Requirements: Ability to work flexible hours as requested. Ability to push and pull up to 1000 lbs with the use of pallet jack. Ability to lift 50 lbs. Must have valid driver’s license and maintain approval from CITYSQUARE’s insurance carrier to operate necessary vehicles.
Position: Community Health Navigator
Department: Project Access
Status: Full-Time

POSITION SUMMARY
The Community Health Navigator - Standard (CHN-S) is responsible for working with and managing patients enrolled in the Project Access Dallas (PAD) program, a network of volunteer physicians, hospitals, pharmacies and ancillary providers, with the goal of providing healthcare services to the uninsured working poor in Dallas County. The CHN-S will specifically provide support to the Community Care Coordination (CCC) program within Project Access. The goal of this program is to help patients achieve independence in managing their own healthcare needs. He/she will conduct interviews and basic clinical evaluations of each patient to determine their clinical and social needs. He/she will teach patients how to properly manage their chronic illnesses and resolve acute medical conditions through health education and patient empowerment. The CHN-S will link patients to appropriate PAD services, provide them with health education and teach them how to find and utilize other medical and community resources. He/she will conduct follow up with providers and patients to ensure resources are utilized effectively.
QUALIFICATIONS
Education: High school diploma or GED required. Completion of a qualified Medical Assistant program required OR Texas Community Health Worker Certification (preferred).
Experience: One (1) year of experience in a similar role within a clinical setting required.
Additional Requirements: Bilingual in English/Spanish required. Must have basic computer skills. Must have valid driver’s license and reliable transportation. Must be able to work occasional flexible hours as requested. Must be able to travel to/from office location to conduct patient home visits.
Position: Workforce Advocate
Department: TRAC (Fort-Worth)
Status: Full-Time

POSITION SUMMARY
The Workforce Advocate will provide specialized case management and coaching to current and former foster youth to assist them in defining their education and vocational goals, supplying them with the knowledge and resources necessary to secure employment opportunities (consistent with their interests, skills, and goals) and to maximize job retention.He/she will work closely with primary case managers to assess each youth for job readiness and locate job training programs and positions that match each youth’s unique interests and abilities. The Workforce Advocate will be the liaison to all workforce programs offered by all four Texas Workforce Boards in the North Texas Region and to job training partners. He/She will refer all youth to appropriate workforce service providers; monitor WIA services and progress; give feedback to the youth’s primary case manager; and monitor youth placed in employment or in self-sufficiency job training programs. He/she will be responsible for building effective partnerships with employers in order to provide current and former foster youth with livable wage employment opportunities. He/she will orient employers to the specific needs of TRAC youth and will work with employers and training partners to ensure job readiness. He/she will be responsible for coordination with the Texas Workforce Solutions network, local businesses and training providers, local public school systems, alternative schools, and colleges for workforce, employment, training, and education services for foster youth.
QUALIFICATIONS
Education: Bachelor’s degree required; degree in social work or a related field preferred; Master’s in Social Work (MSW) highly desirable.
Experience: Two (2) years of experience working with young at-risk populations, youth in foster care or who have been neglected or abused; experience in workforce training or recruitment highly desirable.
Additional Requirements: Must be proficient in MS Word, Excel and Outlook. Must be able to managing a case tracking system. Must be available to work evenings, weekends, and on-call (24 hrs). Must have driver’s license and reliable transportation. Must be able to travel within assigned region on a daily basis. General knowledge of workforce services; an understanding of the array of local services available to support foster youth; a working knowledge of the policies, rules, and procedures of the Texas Department of Family and Protective Services, The Texas Workforce Commission, and other entities providing services to foster youth.

POSITION SUMMARY
The Director of WorkPaths is responsible for overseeing the WorkPaths Department at CitySquare. The Director’s primary responsibility is to build effective partnerships with employers, training institutions, colleges and universities throughout the community in order to provide low income and minority communities with job training and employment. He/she will create and execute a training and employment strategy that will successfully meet the needs of both the employers and the unemployed. The Director will oversee the training and job placement process; from candidate recruitment, to job training, to job placement. He/she will negotiate and manage the contracts held with training institutions and schools. He/she will also supervise the day-to-day operations of the department which includes various administrative and operational responsibilities.
QUALIFICATIONS
Education/Experience: High school diploma plus five (5) years experience in workforce development OR bachelor’s degree plus three (3) years experience in workforce development.
Additional Requirements: Proficient in Microsoft Office applications. Must have a valid driver’s license and reliable transportation. Must be able to work occasional over-time and flexible hours as requested
Position: Mentor Services Manager
Department: TRAC
Status: Full-Time

POSITION SUMMARY
The Mentor Services Manager is responsible for managing the Mentor Program at TRAC. He/she is responsible for ensuring that the initiatives of the Mentor Program are met. He/she is responsible for screening prospective mentors and youth, conducting and overseeing mentor-youth matches and working closely with mentors and youth to help them build and maintain strong relationships. The Mentor Services Manager is responsible for conducting mentor orientation and training. He/she is responsible for assessing the needs of TRAC’s mentors, determining if additional training is needed, and developing curriculum based on these identified needs. The Mentor Services Manager is responsible for providing feedback, via period reports and other documents, to communicate the progress of the youth engaged in this program and he/she will handle other administrative duties, as assigned.
QUALIFICATIONS
Education: Bachelor’s degree in Social Work, Psychology or related field.
Experience: Three (3) years experience working with adolescents and/or families. Experience working with foster families or youth preferred. Two (2) years of volunteer management experience preferred.

Position: Office Representative II

POSITION SUMMARY
The Office Representative II is responsible for checking patients in and out of the clinic, scheduling appointments, and ensuring patients have accurately completed their paperwork. He/she will manage the flow of patients in and out of the clinic; ensuring patients are seen in a timely manner. The Office Representative II is responsible for completing medical reports and assisting the Operations Manager with various administrative duties. The Office Representative II is responsible for documenting No Shows and cancellations in charts and on Misys. He/she is also responsible for scheduling lab appointments, in conjunction with CitySquare Project Access Program. This person, along with the other Office Representatives, is also responsible for all medical record duties. The Office Representative II is responsible for Project Access re-enrollments as well as receiving, verifying and uploading financials to the Project Access Website for updates. He/she will also be the customer service representative touching base with patients to ensure their visits were to their satisfaction.
QUALIFICATIONS
Education: High School Diploma or GED required. Completion of a qualified MA program preferred.
Experience: One (1) year of experience in front office duties, medical setting preferred.
Additional Requirements: Must be bilingual in English/Spanish. Must be proficient in Microsoft Word, Excel and Outlook. Must be able to work over-time or flexible hours as requested.

POSITION SUMMARY
The Medical Assistant is responsible for providing medical assistance to the physicians/care providers at the Community Health Center by performing various patient care duties. He/she will prepare patients for examination and treatment, assist the physicians/care providers with procedures, perform laboratory tests and functions, and educate patients on their plan of treatment. The Medical Assistant is also responsible for cleaning and preparing the examination rooms, ordering supplies, performing quality control and safety procedures, among other responsibilities.
QUALIFICATIONS
Education: High school diploma or GED required. Certificate of Completion as a Medical Assistant from a recognized school required.
Experience: Six (6) months of experience in a clinic setting preferred.
Licensure: Registered Medical Assistant or Certified Medical Assistant preferred.
Additional Requirements: Bilingual in English/Spanish required. Basic knowledge of computers and data entry. Ability to work occasional flexible hours as requested.
