CitySquare’s - OnTRAC Transitional Housing for homeless young adults 18-24. Two levels of residential and supportive services (detailed below) prepare and support young adults as they develop and practice skills to live independently.
Eligibility: Serves homeless young adults 18-24 years old who are leaving or being discharged from the foster care system or other juvenile facilities and lack the resources and support networks needed to obtain housing.
Other important information:
Residents at each program level will enter into a contractual agreement with TRAC staff establishing rules and expectations of the program participant. A primary expectation of OnTRAC program transitional housing residents is that they will utilize the full range of services available to them at the Transition Resource Action Center (TRAC).
The shared apartment housing option allows the young adults to have a peer support network in their apartment and/or apartment complex and will allow them to experience a community living situation, in their apartment and in the apartment complex.
TRAC offers two levels of transitional housing, each proposed to fit the needs of participants who are at varying stages in their journey to independent living. These program options will facilitate the movement of homeless young adults to sustainable independent living within 24 months.
Level I transitional apartments focus on intake, assessment and stabilization. The young adults will share the apartment with three other young adults under the on-site supervision of a resident advisor for up to three months. As they become stabilized and plan a direction for their transition, they will be ready to move into their own apartment Level II, for a maximum of 12 months, indirect supervision will also be offered, but at a reduced level.
The TRAC transitional housing is combined with supportive services that will help promote residential stability, increase independent living skills, and support advancement to livable wage jobs.
